Previous employer mucking me about with SU001 form

Hi,

I had a 12 month contract with a large organisation. My contract finished in April while I was in hospital (ongoing medical issue, related to disability). I didn't get a SU001 then because I was unwell and then focused on recovery.

Now I need an SU001 to apply for Jobseeker and a lot of the people I used to know have gone because there's a restructure and I'm also being told there's no one to do the form and other excuses. But they have a massive HR department and I can't be the only person who's ever asked for a SU001.

I have my final payslip from April and my offer of a fixed term contract from back in 2023 which seem to have all the information needed for the SU001.

Should I go back to the organisation and have another crack? From the Centrelink website they have an obligation to do the form. The website says my employer have up to fortnight to do this and I don't know how I'm going to cover all my costs if this keeps dragging on and I can't get payment.

Or can I submit my contract and final payslip instead?
