Bug report

In chapter 4 I completed all 3 hand scans in the yarnaby section, opened a new gate, and died. When my checkpoint reloads it says I need to complete the 3 hands scans, but they are already done. The door I had opened to move on will not open again. I seem to be stuck in a glitch
