Blood bank not reporting

Hello, I’m not sure what to do and I’m looking for some guidance from fellow MT’s. I work at a large community hospital just outside of Boston. Over the past 2 years the quality of the working being done in our blood bank is going down the toilet. I’m at the point where I’m physically ill over it.

To make a long story short we’ve had many issues that largely involve our SOP not being followed and patients receiving products that are issued on incomplete works up or even worse incompatible!

Specifically we’ve had 2 recent MTP’s on known “B” patients. In both cases the tech gave out previously thawed “A” plasma. (This is very clear in our SOP if the ordering MD cannot wait for compatible plasma that they sign an emergency release and a pathologist is notified).

I have tried to talk about the issue with my supervisor, our head pathologist and lab director. The later 2 were over email and they did not respond. I now feel like I’m being retaliated against (major attitude/ not allowed to pick up shifts). This is not my main concern… the patients are my main concern.

I’m just stuck if I should keep pushing the issue up (and if yes, who outside my hospital?). Or do I just leave and F it. At this point I think it’s impacting my health because I’m so worried about patient safety.
